Let's say the problem with the defense was a communication problem. My question is after numerous practices and scrimmages how and why would that still be an issue. What's going in practice?

I think it's partly about the defensive players recognizing the offensive playcalls and relying on coaches too much for adjustments and reads.

The answer to your question may be in answering this question: who's been on the field in practice, as compared to who was on the field during the game? And don't tell me it shouldn't matter; leadership and communication patterns change as personnel change, and taking out 5 of the more important members of your defensive rotation - will change those patterns significantly.
