Anyone else get a questionnaire from IU Athletics today?

I received a questionnaire from IU inquiring about my fan experience yesterday at the Rutgers game. I have been a season ticketholder since 2005 and to my recollection( I am old like many and subject to the occasional memory lapse) have not received such a questionnaire before. I will include it below. I think something is up and the "game is afoot." IMO there are 3 questions of value and the rest are all fluff.
